What each form collects

The Reach Out form collects your name, email address, interests, and any note you choose to add. Please do not use it for health or medical details.

The Client Intake collects identity and contact details, date of birth, emergency contact, skincare goals and routine, relevant health history, allergies, medications, prior treatments, lifestyle answers, and your consents.

The Treatment Consent & Waiver collects identity and contact details, date of birth, selected services, acknowledgments, your private-record photo choice, and electronic signatures. Submission identifiers, timestamps, form versions, and email-delivery status are also recorded.

How it is used

Inquiry details are used to respond and send your confirmation after you provide the required processing consent. Occasional studio updates are sent only when you select the separate optional updates checkbox. Intake and waiver details are used to plan services, identify matters to discuss before treatment, maintain the client record, and document your choices and consent.

Swan Esthetics does not use intake or waiver answers for advertising and does not sell form information.

Where it goes

OpenAI Sites hosts this website and processes form submissions so they can reach Swan Esthetics' form service. Responses are stored in a restricted Google client-record tracker accessible only to the studio owner and an authorized support person. Cloudflare Turnstile processes technical request information to screen spam.

Resend delivers outbound email. For intake and waiver submissions, the completed PDF goes only to the email address you enter and confirm. A separate notice goes to bookings@swanesthetics.studio with your name, form type, submission time, and reference number—but not your phone number, health history, signatures, or form answers.

Reach Out details are sent to hello@swanesthetics.studio, and a confirmation is sent to your email address. Those messages are also handled by the sender's and recipient's email providers.

Your choices

You may leave optional fields blank and can ask to discuss a sensitive detail in person. Do not submit a form if you do not agree to the stated processing and email delivery. You can submit a Reach Out inquiry without signing up for studio updates. Choosing No for private-record photos does not affect your service; marketing use requires separate permission.

To correct information, request a copy, unsubscribe from updates, or ask Swan Esthetics to delete a record, email hello@swanesthetics.studio. Swan Esthetics will review the request and explain if a record must be retained for an existing appointment, transaction, safety, or legal obligation.

Keeping copies private

Website forms use an encrypted connection. Email is not a private record vault, so protect the completed PDF and the email account that receives it. If email delivery is not right for you, contact the studio before submitting the intake or waiver to discuss another option.
